Family of Faith Service Providers
This service is provided to the individual by a matched family for long term care. This service is provided 365 days a year in the home of a host family. Family of Faith can pay a family member to provide the service as long as the family member is 18 and resides in the same home of the individual.
Host Home Companion Care
This service may be rendered if the individual lives alone or with a family member. A staff member will assist for a specific number of hours per day or week.
A lifetime amount of $7,500.00 is available for minor home modifications. A physical therapy assessment to determine need is required
Respite can be provided for a few hours or a few days for individuals who live at home.
INDIVIDUALS LIVING IN A RESIDENCE WITH 24 HOUR DIRECT CARE STAFF. STAFF ARE RESPONSIBLE FOR CARING FOR THE DAILY LIVING NEEDS OF THE INDIVIDUAL TO ENSURE A LEAST RESTRICTIVE ENVIRONMENT CHOICE OF LIVING CONTINUES.
