Services


Respite Relief (with CFC option only)-



This service may be rendered if the individual lives alone or with a family member. A staff member will assist for a specific number of hours per day or week.   


 

This service is provided to the individual by a matched family for long term care. This service is provided 365 days a year in the home of a host family. Family of Faith can pay a family member to provide the service as long as the family member is 18 and resides in the same home of the individual.



Respite can be provided for a few hours or a few days for individuals who live at home.



Physical therapy, Occupational therapy, Speech therapy, Dietary services, Audiology, Psychology, Day Habilitation, and Nursing



Adults over the age of 21 will receive $1000.00 per year for dental care. Adaptive aids- Any aids that are not covered by Medicaid.  Adaptive aids can include lifts, walkers, wheelchair ramps, diapers, etc.


 

A lifetime amount of $7,500.00 is available for minor home modifications. A physical therapy assessment to determine need is required.



BS provides specialized interventions that assist an individual to increase adaptive behaviors that replace socially unacceptable behaviors that may interfere with the individual’s inclusion in their family home life or community life.

Family of Faith is a faith based agency that provides services to individuals with Intellectual and Developmental disabilities. 

Family of Faith was developed to provide customized services that will enable individuals of all ages to reach their goals, move towards their greatest potential, and integrate into the community. Family of Faith desires and consistently prays that God continue to equip us to build the kingdom by impacting the lives of people we serve.


How we serve:

All individuals and their family members are considered important to Family of Faith and we are honored to have them be a part of an integral team. They are the driving force in building and implementing their person directed service plan. Our support, assistance, and guidance will assist in making decisions easy, and the desires of the individual is always our first priority.



Family of Faith HCS:

All individuals will have a Family of Faith Case Manager that will assist with coordinating all services. A phone contact will be rendered monthly and home visits as needed. To receive services all individuals must have an IQ of 75 or below and be eligible for Medicaid.
